Please read the descriptions below and send your request through the appropriate channel.

Records request: If you want to examine or receive a copy of an existing document or record, you may submit a public records request as described in the Oregon Revised Statue (ORS) Chapter 192 — Records; Public Reports and Meetings. Please submit a public records request by following the instructions provided below.

Making a records request
A public records request must be made in writing. Please complete a public records request form. Be sure to include all of the following:
- Requestor's name, address and phone number
- Specific information requested
- Records from which the information is requested, if known
- Format in which the information is requested

Request process
As a public utility owned by the citizens of Eugene, EWEB strives for transparency and will make efforts to mitigate barriers for those seeking public records. We also have a responsibility to comply with public records requests in a manner that minimizes costs to all customer-owners.
Oregon law (ORS 192.324(4) and (5)) provides that the EWEB may establish cost-recovery charges reasonably calculated to reimburse EWEB for the actual costs of making public records available. Read more about the request process and charges.
We will consider requests for waivers or a reduction of charges in instances where EWEB deems the release of public records primarily benefits the general public. To apply for a waiver or reduction of charges, complete and submit the waiver of charges form.
Liability Claims
A liability claim may be made by third parties to request reimbursement for losses related to injury or damages allegedly resulting from EWEB operations.
As a self-insured public entity, EWEB can only pay those claims for which we are legally liable. For EWEB to be considered responsible for damages, EWEB must be proven to have been negligent.
The Oregon Tort Claims Act generally exempts public bodies such as EWEB from liability resulting from the performance or non-performance of discretionary functions. For example, due to resource limitations EWEB has limited capability to physically inspect many parts of our electric or water distribution system. Such work is considered discretionary, and in most cases EWEB is exempt from liabilities for losses related to such discretionary work or conditions.
Filing a Claim
To file a liability claim, please download and complete the Claim Form and return to EWEB.
Alternatively, please contact EWEB Risk Analysts at EWEB.Claims@EWEB.ORG or call 541-685-7494 and leave a message with contact information. An EWEB Risk Analyst will respond and help you through the claims process.
Once we have detailed information, a Risk Analyst will investigate and thoroughly evaluate the facts of the claim. If evidence proves that EWEB was negligent in its duties, EWEB may consider reimbursement of the claimed losses pursuant to Oregon Law. If facts demonstrate that EWEB was not negligent, payment for the claim would be denied.
